Locations provide:

✅ Medication reminders (if needed)
✅ Transportation coordination
✅ Life skills support
✅ Coordination with case managers and resource agencies
✅ Supportive environment focused on stability and independence

Applicants welcomed from:
* Self
* Hospitals
* Case Managers
* Social Workers
* Behavioral Health Programs
* Reentry Programs
* Community Organizations
* Managed Care & CalAIM Partners
* Other Care Professionals 

Applicants should be:
Senior, or
Low income, or
Permanently disabled
