Homeless to Housing: Stability and Care

We are dedicated to helping adults move toward a future of stability and independence.

A Complete Solution: Our wonderful partners provide a supportive environment: safe, clean living spaces with all utilities included, plus helpful extras like transportation assistance, on-site case management, and furnished rooms. To ensure everyone feels at home, they also provide three nutritious daily meals and valuable life skills support.

Compassionate Care: We coordinate with case managers to ensure you receive the medication reminders and structural support you need.

Who can apply: If you are a hospital case manager, social worker, behavioral health case manager, re-rentry program case manager, community organizaiton case manager or anyone working with a senior/care professional, please reach out to us.

How to Qualify: We are honored to serve our senior neighbors, older adults living with permanent disabilities, and individuals managing low-income challenges.
